What do you do on the daily basis as a doctor?
As a doctor? Well, my work is helping patients modify their lifestyle. That involves a lot of talking, motivating, and skill training. So I limit seeing not more than 4 patients a day. It usually starts with face-to-face meeting, I mean history taking, physical examination, order some labs and images, then summarize that patient’s health risks. Then together, I mean doctor and patient, we draft out health plan for one year with target goal for each risk. Then we decide what skill is required to achieve such goals. The time after that first meeting will be a training and checking period, mostly done via telephone conversation. Sometimes I made appointment for some patients to come in for particular skill training, such as strength training exercise, cooking methods, stress management technique etc. So, in addition to sitting in the clinic room, I often do exercise teaching in gym, do meditation teaching, or even run cooking class.
คุณทำอะไรในแต่ละวันในฐานะที่เป็นหมอ?

          ในฐานะที่เป็นหมอเหรอ ก็.. งานของผมคือช่วยคนไข้ให้เปลี่ยนวิธีใช้ชีวิตของเขา มันก็ต้องพูดกันแยะ กระตุ้นกันแยะ และสอนทักษะกันด้วย ดังนั้นวันหนึ่งๆผมรับดูคนไข้ไม่เกินสี่คน มันจะเริ่มด้วยการนั่งคุยกันซึ่งๆหน้าก่อน ผมหมายถึงการซักประวัติ ตรวจร่างกาย สั่งแล็บ ตรวจด้วยภาพต่างๆ แล้วสรุปออกมาว่าคนไข้คนนั้นมีปัจจัยเสี่ยงสุขภาพอะไรบ้าง แล้วทั้งสองฝ่าย หมายความว่าผมกับคนไข้ ก็จะมาร่างแผนที่จะดูแลตัวเองเป็นเวลาหนึ่งปีจากนั้นไปร่วมกัน ในแผนนั้นก็จะมีตัวชี้วัดและเป้าหมายด้วย ระยะเวลาหลังการพบกันครั้งแรกก็เป็นช่วงของการฝึกทักษะและติดตามตรวจสอบ ซึ่งส่วนใหญ่ก็เป็นการคุยกันทางโทรศัพท์ บางครั้ง ผมอาจจะนัดคนไข้บางคนมาฝึกทักษะในบางเรื่อง อย่างเช่นการสร้างความแข็งแรงของกล้ามเนื้อ การทำอาหาร การจัดการความเครียด เป็นต้น ดังนั้นนอกจากจะนั่งอยู่ในห้องตรวจโรคแล้ว บาเวลาผมก็สอนออกกำลังกายอยู่ในห้องยิม บางเวลาผมก็สอนทำสมาธิ หรือแม้กระทั่งสอนวิธีทำครัวอยู่ในครัวก็มี
Why did you choose this specialty?
      Because I was sick. Ha ha.. that’s right. It’s a long story. Previously I worked as a cardiac surgeon doing bypass operation for quite sometimes, say 20 years. Then I became a sick man, with hypertension, hypercholesterolemia and coronary heart disease. So I did literature review to seek for ways to treat myself, any possible way that I don’t have to operate on my own heart. My research convinced me that the only way to go is to modify my lifestyle totally. So I did it, and succeeded in doing that. I mean I cured myself from hypertension, dyslipidemia and reduce my weight for instant. So one bright idea popped up in my head
“Hey, this is cool man. Why work hard. Why doing bypass operation day in day out? Why don’t you just teach patients to modify their lifestyle so that they will not end up having bypass operations.”
 So, at fifty-something, I stopped doing heart operation, went back to study and did clinical research to become a qualified family physician. So, here I am.
ทำไมคุณถึงเลือกเป็นหมอสาขานี้?

          ก็เพราะว่าผมป่วยนะซิ ฮะ.ฮ่า ใช่แล้ว เรื่องมันยาว ก่อนหน้านี้ผมเป็นหมอผ่าตัดหัวใจ ทำผ่าตัดบายพาสมาพักหนึ่ง ก็กว่ายี่สิบปีแล้วแหละ แล้วผมก็กลายเป็นคนป่วยซะเอง ความดันสูง ไขมันในเลือดสูง และเป็นโรคหลอดเลือดหัวใจ ผมกลับไปทบทวนวรรณกรรมเพื่อเสาะหาวิธีรักษาตัวเอง แบบว่า ไม่ต้องผ่าตัดหัวใจให้ตัวเองหนะ งานค้นคว้าครั้งนั้นตอกย้ำกับผมว่าวิธีไปเหลืออยู่ทางเดียว คือผมต้องปรับวิธีใช้ชีวิตของผมไปเสียอย่างสิ้นเชิง ผมก็.. ลงมือเลย และประสบความสำเร็จเสียด้วย ผมหมายความว่าความดันเลือดสูงหาย ไขมันในเลือดกลับมาปกติ และพุงยุบ เป็นต้น ฉับพลันนั้นผมก็ปิ๊งไอเดียนี้ขึ้นมาว่า
          “เฮ้ย..ลุง จะมามัวทำงานหนักผ่าตัดหลอดเลือดหัวใจวันแล้ววันเล่าอยู่ทำไมเล่า ทำไมลุงไม่สอนคนไข้ให้เขาปรับการใช้ชีวิตของเขาเอาเพื่อที่จะได้ไม่ต้องมาผ่าตัดจะไม่ดีกว่าหรือ”
          ดังนั้น ณ อายุห้าสิบกว่า ผมก็เลยหยุดผ่าตัดหัวใจ กลับไปเรียนใหม่ ทำวิจัยในสาขาใหม่ แล้วสอบเป็นผู้เชี่ยวชาญสาขาเวชศาสตร์ครอบครัว แล้วจึงได้มานั่งอยู่นี่ไง
Why do you love your job so much?
Firstly, it is much lighter than my previous laborious job as a cardiac surgeon. Ha ha.
Secondly, in doing this job I also teach in order to learn things myself. For instant, currently I focus on using MBSR (mindfulness based stress reduction) to manage many chronic incurable illness, in doing this I myself learned to use mindfulness methodology faster than what I ever did in my previous 30 years of practice. So doing this job I am improving my own life as well. That’s why I love this job.
ดูคุณจะชอบงานนี้มากนะ..ทำไมหรือ?

          ข้อแรก มันเบากว่าจ๊อบเก่าผ่าตัดหัวใจนซึ่งต้องออกแขกแบบหามหนักกว่ากันแยะ ฮ่า..ฮ่า
ข้อสอง เวลาทำงานนี้เนี่ย ผมก็ได้เรียนรู้สิ่งต่างๆด้วยนะ อย่างเช่นตอนนี้ผมกำลังโฟกัสที่การเอาฝึกสติลดความเครียด (MBSR) มาจัดการโรคเรื้อรังที่รักษาไม่หายสารพัด การทำสิ่งนี้ทำให้ผมมีความก้าวหน้าในฝึกสติของตัวเองเร็วกว่าช่วงชีวิตก่อนหน้านี้สามสิบปีเสียอีก ดังนั้นทำงานแบบนี้ผมได้พัฒนาชีวิตตัวเองไปด้วย นั่นเป็นเหตุผลที่ทำให้ผมชอบงานนี้
What is your working motto?
My motto has been changing with time, you know? Previously I was a man who loved to multitasking things. I did heart operation and at the same time ran hospital business, as a hospital director, as well. But now when I get old my working motto is..
          “..One thing at the time,
            And if I can’t get all things done in time
            I won’t give a damn..”
Ha ha ha, and I can assure you, it works!
คุณมีม็อตโต้ในการทำงานว่าอย่างไร?

          ม็อตโต้ในการทำงานของผมเปลี่ยนไปเรื่อย แต่ก่อนผมชอบทำหลายอย่างพร้อมๆกัน เป็นหมอผ่าตัดหัวใจด้วย เป็นผู้อำนวยการบริหารธุรกิจของโรงพยาบาลไปด้วย แต่เดี๋ยวนี้พอแก่ตัวลง มอตโต้ในการทำงานของผมคือ

          “ทำที่ละอย่างเถอะน้องเอ๋ย
            และถ้าเอ็งไม่สามารถทำทุกอย่างได้เสร็จทันในเวลา
            ก็…ช่างแม่ม..ม ปะไร”

 ฮะ ฮะ ฮ่า แล้วผมจะบอกคุณนะ มันได้ผลนะจะบอกให้
Tell me your most difficult case that you have been as a doctor?
Let’s talk about only when I am a family physician, not when I was a cardiac surgeon, O.K?
In changing behavior or lifestyle modification, every patient is difficult case. So, I’d better talk about how I overcome such difficulties. Initially, I focused on providing knowledge. Unfold major medical research to them in lay-man’s term. Yeh, it seems enlightened them but knowledge has very limited power in changing people’s behavior. Then I learned from some successful cases that in those successful cases, they were motivated by either pain of fear. Admittedly, at one stage of my practice, I used lab or imaging evidence to postulate fearful outcomes to motivate my patients. For instant, I displayed image of their calcified coronary arteries to convince them that they already have the disease, then talk about the subsequent blockage and heart attack. It worked for some extend. But despite such dreadful facts, still a lot of patients cannot change the way they response to daily stimuli, because the tracts in their brain had been repeated so often that those tracts become so deep and so permanent. As I mentioned, currently I work with MBSR, I mean mindfulness based stress reduction. I am using MBSR to change health behaviors. My idea is that in lifestyle modification you depend on self-motivation, which actually is self-discipline. And you certainly cannot impose self-discipline when you are not mindful or not being aware of your current state of mind.
In medicine, we already have tons of evidence supporting using MBSR in reducing chronic symptom and improving outcomes of chronic diseases. But there is limited evidence concerning using MBSR in lifestyle modification. It is an interesting research front, and it is good fun.  
ช่วยเล่าเคสผู้ป่วยที่ยากๆที่คุณเจอให้ฟังหน่อยสิ?

เราคุยกันถึงเคสเมื่อผมเป็นหมอประจำครอบครัวดีกว่านะ ไม่ใช่เมื่อผมเป็นหมอผ่าตัดหัวใจ โอเค.นะ

ในการเปลี่ยนวิธีใช้ชีวิตของผู้คนเนี่ย คนไข้ทุกคนเป็นเคสที่ยากหมด ดังนั้นผมจะเปลี่ยนมุมมาพูดถึงว่าผมเอาชนะความยากนั้นได้ไงดีกว่า ตอนเริ่มต้นทำงานนี้ ผมโฟกัสที่การให้ความรู้ กางแผ่ผลวิจัยทางการแพทย์แบหราต่อหน้าพวกเขา ในภาษาคนธรรมดาที่พวกเขาเข้าใจด้วยนะ ช่าย..ย ดูเหมือนพวกเขาฟังแล้วจะบรรลุธรรม แต่ว่าเอาเข้าจริงๆความรู้มันมีพลังที่นำไปสู่การเปลี่ยนแปลงพฤติกรรมน้อยมาก ต่อๆมาผมก็สังเกตเอาจากเคสที่ประสบความสำเร็จ ผมพบว่ามันสำเร็จเพราะความเจ็บปวดทรมานจากโรคที่เป็น หรือไม่ก็เพราความกลัว ผมยอมรับเลยนะว่าช่วงแรกๆที่ผมทำงานนี้ผมใช้ผลแล็บหรือไม่ก็ภาพเอ็กซเรย์คอมพิวเตอร์มาวาดภาพอนาคตที่แย่ๆให้เขาเห็นเพื่อใช้เป็นเครื่องมือกระตุ้นคนไข้  อย่างเช่นผมแสดงภาพแคลเซียมที่หลอดเลือดหัวใจของเขาบนจอคอมพิวเตอร์แล้วย้ำว่าเขามีโรคเรียบร้อยแล้วนะ แล้วก็คุยต่อถึงว่าโรคมันจะดำเนินต่อไปอย่างไร วิธีนี้มันก็เวอร์คบ้าง ไม่เวอร์คบ้าง เพราะไม่ว่าความจริงที่เล่าให้ฟังจะน่ากลัวเพียงใด แต่คนไข้จำนวนหนึ่งก็ไม่สามารถเปลี่ยนแปลงวิธีที่จิตของเขาสนองตอบต่อสิ่งกระตุ้นในชีวิตประจำวันได้ เพราะร่องวิธีการสนองตอบแบบเดิมในสมองเขามันเป็นร่องลึกที่เขาสนองตอบแบบนั้นซ้ำๆซากๆจนยากที่จะเปลี่ยนได้ อย่างที่ผมเปรยไปแล้ว ตอนนี้ผมกำลังทำงานอยู่กับการฝึกสติเพื่อลดความเครียด (MBSR) ผมกำลังใช้ MBSR มาเปลี่ยนพฤติกรรมการใช้ชีวิต ความคิดของผมก็ในคือการปรับวิธีใช้ชีวิตเนี่ย คุณต้องอาศัยแรงกระตุ้นหรือความบันดาลใจ ซึ่งจริงๆแล้วก็คือวินัยต่อตนเองนั่นเอง แล้วคุณจะมีวินัยต่อตนเองได้อย่างไรถ้าคุณลืมตัว ไม่มีสติ คือไม่รู้ว่า ณ ขณะนั้นจิตของคุณกำลังทำอะไรอยู่ที่ไหน      
ในวงการแพทย์เนี่ย ตอนนี้เรามีหลักฐานวิทยาศาสตร์เป็นตัน มากเกินพอที่จะสนับสนุนให้ใช้ MBSR ในการลดอาการและทำให้จุดจบของโรคเรื้อรังต่างๆดีขึ้น แต่ยังไม่มีหลักฐานเรื่องการใช้ MBSR ในการปรับวิธีใช้ชีวิตมากนัก มันเป็นแนวรบการวิจัยที่น่าสนใจ และมันก็..สนุกด้วยนะ
